I put a Linux mail-relay machine between the world and the exchange
server, and route all mail through it. THen I use Spamassassin
(http://www.spamassassin.org), and John Hardin's mail sanitizer
(http://www.impsec.org) to kill most worms before they make it to
exchange. John even has a howto on protecting exchange servers with
Linux/Sendmail.
You can even get some cool anti-virus tie-ins to scan for virii on the
relay box...thus saving processing time one the exchange box itself.
