You nailed it.  It's a hell of a big project, but see koha.org for one 
approach to the problem.  Note that Koha has several shortcomings that make 
it currently suitable for only a small library, but it's moving in the 
right direction.

Proprietary solutions often incorporate code and/or architecture that goes 
back five years or more.  We're currently using a system that was ported 
from god-knows-what, and is currently running in (among other things) PICK 
BASIC on a UniVerse db on an HP-UX machine.  It uses proprietary protocols 
to communicate with Public Access and Cataloging programs.
