a quick google of Debian & LVM issues found this:
https://bugs.debian.org/cgi-bin/bugreport.cgi?bug=616689

so it may BE a real Debian issue...

and here are some standard caveats for LVM installation:

http://serverfault.com/questions/279571/lvm-dangers-and-caveats

any of these could have been his issue..

Risks of using LVM:

  * Vulnerable to write caching due to hard drive/SSD or VM hypervisor
    (or old Linux kernels)
  * Harder to recover data due to more complex on-disk structures
  * Harder to resize filesystems correctly
  * Snapshots are hard to use, slow and buggy
  * Requires some skill to configure correctly given these issues
