A scam seems unlikely, IMO. But I might get verbal confirmation of the
URL/email address from the person I spoke with earlier before
proceeding. Don't trust phone numbers in the email.
Even though what they're collecting isn't terribly confidential, it
seems like bad form to me to use an embedded HTML form in an email. A
secured website link would have been better.
