I used bellsouth dial up for a while.  Here is my /etc/wvdial.conf.  Sanitized
of course.

Modem = /dev/ttyS0
Baud = 115200
Init1 = ATZ
Init2 = ATQ0 V1 E1 S0=0 &C1 &D2 +FCLASS=0
ISDN = 0
Modem Type = Analog Modem
Dial Prefix = *70,
Phone = 678-309-0607
Username = ELN/<username>
Password = <password>
