I have to second, third and fourth all the warnings against Matt and his
CGI...he's to be avoided at all costs.

One thing I would add is when you're looking for a script, look for one
that says it validates user input (ie, that the data it sends to you is
checked to make sure it's the kind of data you're expecting).  That
eliminates a huge percentage of nasties with web forms.  Unfortunately I
write all my forms myself so don't know of any to download but maybe some
Alers do.
