Well for one thing I see you are trying to modprobe i2c_proc
when the actual module name is i2c-proc

Without the "path[i2c]=/lib/modules/`uname -r`/kernel/drivers/i2c"
in the modules.conf file and then run depmod -a are there any
entries in the modules.dep file for the kernel that you are using?
