I'm doing it right now, screening my fax line for calls.  Mine does call
a perl script, so that's not the problem.  Here's my entry in the
config:

cnd-program /home/esoteric/shl/callerid.pl

Nothing to it from there, the parms are passed automagically.

I'm headed that way too, but I've not got all my modems into one machine
as I want just yet.
