^^^ This.  The only time this has ever happened to me has been a broken
initramfs.  Core kernel code doesn't understand labels or uuids.  If the
initramfs is broken and the kernel tries to interpret root= by itself,
only the true device names will work.  Then, on modern distros, a bunch
of things udev sets up in the initramfs don't get done, screwing up the
rest of boot.
