In the past, I have bought unlocked GSM phones from Amazon, Ericsson
(directly), 800mobiles.com (or something like that), and Ebay. I've
never had a problem with any of them.
As to a particular phone which is cheap and simple, I highly recommend
the old T-39m. It's an old model, but it has excellent battery life
(a week to ten days of my use... ~3000min/month), Bluetooth, excellent
call handling, very slim design, and good reception. It is also
tri-band; it will work in the US or the rest of the world. The only
reason I'm not using one now is because I "upgraded" to a G1.
