No.  I'm currently using a 20" Samsung flat and a 22" ViewSonic monitor. 
Works fine.  I have used a 20" viewsonic with a 16" dell.

Should be no problem.

I've never seen one that has more than two. (not counting the tv out) 
You can opt for the supported ATI cards, use Linux open drivers and 
xinerama, or you can go with the nVidia cards and use their drivers.  I 
think you need to use the nVidia drivers to do multiple desktops though.

Depending on your mb, you'll likely need a pci card and an agp card. 
I've never done multi-head with anything other then a single card that 
has two outputs.
