I'm using a combination of sendmail plus three filters -- but the one 
that's made the biggest difference is milter-gris, which uses a 
technique known as greylisting.   It's reduced the spam to nearly a 
trickle, and that's without applying anything else on top like 
spamassissin or bogofilter.  :)
