We've just started using XPlanner (http://www.xplanner.org/).  It will
definitely meet your first two requirements, not sure about the third,
but it's open source :).  It is designed for a team, but should work
fine for a team of one.

Don't let the XP stuff on the home page throw you.  You don't need to
use XP to benefit from XPlanner; we use it solely for time tracking and
it works well.
